All patients will be required to keep a valid credit/debit card on file. Cards on file will only be charged in the event of a late cancellation (less than 24 hours notice) or missed appointment (no-show). Fees are as follows: late cancelation $30 fee & no-show $60.

If main lot is full, there are two additional parking spots along the front of the building.
