All patients will be required to keep a valid credit/debit card on file. Cards on file will only be charged in the event of a late cancellation (less than 24 hours notice) or missed appointment (no-show). Fees are as follows: late cancellation $30 fee & no-show $60.

Please note that we do NOT accept Medicare or Medicaid Plans, including Medisource, Medicare or Essential Plans from Independent Health, Univera or BlueCross BlueShield (Highmark, Emblem, etc.)

If main lot is full, there are two additional parking spots along the front of the building.
